Considerations for Premium Discount and Surcharges:

Wellness Premium Discount: By completing the Cigna health assessment and biometric screening, employees can earn a preferred premium rate that discounts medical premiums. 

Tobacco Surcharge: a tobacco use surcharge will be applied to medical contribution rates for employees and their dependents who use tobacco products.

Spouse Surcharge: If your spouse/domestic partner has coverage available to them through their employer and you elect to enroll them in Element Fleet coverage, you will be assessed a surcharge on your medical contribution rates.

What are the coverage differences for each plan and why do they have different costs?

League & Element Fleet are excited to share three different health insurance plan offerings that allow you to make the best decision for you and your family. These three plans provide greater flexibility in allowing you to choose what you need from your health insurance. Each plan can be utilized to make the most out of the differing coverage.

Flexible offers the least amount of insurance coverage through Cigna with the lowest payroll deductions. This plan includes in-network deductibles of $3,000 individual / $6,000 family with 20% member coinsurance after deductible. Additionally, Option 1 includes a $750 individual / $1,500 family employer contribution into your Health Savings Account (HSA), with the opportunity to earn additional HSA dollars through the Cigna MotivateMe Program for this plan. 

Hybrid offers moderate coverage with Cigna and has comparable payroll deductions to the current medical plan offerings. This plan includes in-network deductibles of $2,000 individual / $4,000 family with 20% member coinsurance after deductible. Additionally, Option 2 includes a $500 individual / $1,000 family employer contribution into your Health Savings Account (HSA), with the opportunity to earn additional HSA dollars through the Cigna MotivateMe Program for this plan. 

Comprehensive offers the highest amount of medical coverage with Cigna, and therefore has the highest cost associated with it (premium paid through a payroll deduction). This plan includes in-network deductibles of $1,000 individual / $2,000 family with copayments and 20% member coinsurance after deductible. You have the opportunity to earn rewards into a Lifestyle Spending Account (LSA) through the Cigna MotivateMe Program for this plan.

When can I change my plan elections outside of this enrollment time period?

The only time you can change your health insurance coverage is when you experience a Qualifying Life Event like getting married or adding another member to your family!


What is a Health Savings Account?

A Health Savings Account (HSA) is a tax-advantaged account that is administered through Bank of America if you select the Flexible plan or the Hybrid plan (if you are under age 65 or not enrolled in Medicare). You can apply HSA funds toward a wide range of qualified health-related expenses and funds roll over from year to year. HSAs are administered in accordance with US IRS guidelines. Money contributed to this account is on a pre-tax basis!


What is a Health Reimbursement Arrangement?

If you are 65 or older, enrolled in Medicare, and enrolled in the Flexible or Hybrid medical plans, you will receive a Health Reimbursement Arrangement (HRA) instead of the HSA. The HRA is funded by employer contributions only and can be used to pay for qualified healthcare expenses. Any unused funds at the end of the year will be rolled over to the following year. Employees have one year to file a claim for reimbursement for the prior year’s services and still be reimbursed.


How much do I pay for these medical plans and how much is covered by my company?

Your employer covers approximately 85% of the annual premium for the Flexible plan, 75% of the annual premium for the Hybrid plan, and 65% of the annual premium for the Comprehensive plan.

Therefore, you’ll be paying approximately 15% of the annual premium for the Flexible plan, 25% of the annual premium for the Hybrid plan, and 35% of the annual premium for the Comprehensive plan.


Where can I see my payroll deductions?

You can see the payroll deductions while in the enrollment experience making your elections on the League platform. Once you’ve submitted your elections, you’ll be able to see your payroll deductions based on the elections you’ve made on the summary page.


What is a Lifestyle Spending Account and what can I use it for?

If you enroll in the Comprehensive medical plan, you can earn rewards through the Cigna MotivateMe Program that are deposited into the Lifestyle Spending Accounts (LSA). LSA funds can be used to cover services and products that help you live a healthier life such as gym memberships, yoga, meditation, etc. Money spent from this account is a taxable benefit and you only are taxed on the funds you actually use. League is the administrator of your LSA (instead of an insurance carrier) meaning that all claims are submitted directly through your League app.

If you are enrolled in the Flexible or Hybrid medical plans, the rewards you earn through the Cigna MotivateMe Program are deposited into your HSA (or HRA if you are 65 or older and enrolled in Medicare).
